Chantelle currently works in the role of clinical nurse specialist at a local hospice caring for patients in the community; which requires skills in remote triage and remote assessment as well as face to face review and diagnostic knowledge. Chantelle supports patients and their families from diagnosis to post bereavement with all aspects of their care, focusing in particular on advanced care planning and management of symptoms.
Chantelle has previously worked in the Accident and Emergency Department which incorporated a national trauma centre receiving patients from the whole of the West Midlands and Wales. Chantelle has 6 years of experience in the Emergency Department and has gained skills in triage, physical health assessment and minor injuries and illness. Chantelle has worked as part of the district nursing team and progressed to a senior role with the hospital avoidance team and supported the early discharge team completing holistic assessments, preparing treatment plans and reviewing the efficacy of these in achieving the patients’ goals whilst managing and responding to risks.
